Children

For us to refer a child to a specialist, to assess whether they may have Autism Spectrum Disorder (ASD), we first require both of the following forms completed (please click form name to download):

1. 1. Autism assessment form completed by parent/carer
   2. Autism assessment form completed by school

Once we have both completed forms, please book a ROUTINE telephone appointment with a GP to discuss the next steps.

For children under age 3, please liaise with your health visitor.

Adults

To discuss a referral for an adult, please book a ROUTINE telephone appointment with a GP, we do not require forms in advance.
