Update Personal Details

Important Notice

If you change any of your personal details that we hold in your file, please let us know as soon as possible. If we need to contact you for any reason it is important that we have the correct details.  The form below can be used to update the following details:

  • Name
  • Address
  • Telephone number
  • Email address
  • Marital Status
  • Notification consent
  • Other

If your address has changed, please ensure that you are still within the Practice Boundary.

Address Changes – Read This Carefully

If you are updating your address, you must ensure that you still live within our practice boundary.

If your new address is outside the boundary, you cannot remain registered under standard registration rules.

In this situation:

  • You may be changed to an Out of Area (OOA) patient, if clinically appropriate

  • Out of Area registration has different rules, including:

    • No home visits

    • Urgent care may be directed to services near where you live

  • You may be asked to complete an Out of Area waiver

  • In some cases, you may be advised to register with a GP practice closer to your home

Out of Area registration is not automatic and is reviewed for clinical safety and practicality.

If you’re an Out of Area patient at Manor View Practice:

Home Visits

  • We cannot provide home visits because you live outside our boundary.

  • If you’re too unwell to come in, contact NHS 111 for local support.

  • Call 999 in life-threatening emergencies.

Urgent & Emergency Care

  • If you cannot travel to us with an urgent problem, NHS 111 will direct you to local urgent care or A&E.

  • Emergency symptoms (e.g., chest pain, stroke symptoms, severe breathing difficulty) should always be managed by 999.

Routine Appointments

  • You can still book and attend GP and nurse appointments at MVP.

  • Reviews, vaccinations and other routine care take place in the surgery or by phone, not at home.

Community Services

  • Some services such as midwifery, health visiting and district nursing may be provided by the NHS team in the area where you live, not directly by MVP.

Moving Home

  • Tell us promptly if you change address, even within the same broad area.

  • We must reassess whether OOA registration is still appropriate.

Your Health Changes

  • If your medical needs increase, you may be advised to register with a practice nearer your home for safety and continuity of care.

Digital & Online Access

  • You can still use online consultation and digital access tools during core hours (08:00–18:30), in line with the NHS Patient Charter and GP contract.

By signing the OOA waiver, you confirm you have understood and agree to these terms.

Before You Submit

If your address has changed, please check your postcode carefully before submitting the form.
